Polo, found it! The insurance card was in zippered section all along. It was hidden in a quad-folded card I received from the Santa Barbara Zoo listing endangered species of fish that patrons should avoid ordering at restaurants.

"This would only happen to you," hubbie Mark said eyes rolling.

I don't want to leave you hanging so here's the fish list:

Chilean Seabass/Toothfish*

Cod:Atlantic

Crab King (imported)-- go with Alaska or US imitation

Dogfish (US)*

Grenadier/Pacific Roughy

Lobster:Spiny (Caribbean imported)-- order American or Maine

Mahi mahi/Dolphinfish (imported)

Monkfish

Orange Roughy*

Rockfish (trawled)*

Salmon (farmed, including Atlantic)* -- stick to west coast salmon

Scallops: Sea (Mid-Atlantic)-- choose Canada and Northeast Sea, instead

Sharks*

Shrimp (imported farmed or wild) -- stick to US farmed or wild

Sturgeon* Caviar (imported wiled)-- go with OR or WA wild

Swordfish (imported)* -- choose US longline, instead

Tuna: Albacore, Bigeye, Yellowfin (longline)*

Tuna: Bluefin* -- get Bigeye or Yellowfin

*= Limit consumption due to concerns about mercury or other contaminants.
